Position Summary:
General responsibilities include proactively reaching out to members to communicate cost savings opportunities and explain the benefits of Medco’s programs; applying specialized knowledge of drugs and drug therapies to meet the specific needs of members; performing comprehensive evaluations of member health history against current medications to ensure optimal safety and enhance the care and treatment of specific chronic conditions; engaging in the appropriate level of discussions with other health professionals to provide total pharmaceutical care; accountability to team on performance metrics including patient care plans, therapy management guidelines, customer satisfaction, margin, productivity and professional practice standards. This position reports directly to the site’s Sr. Manager of Pharmacy Operations.
This position is located in Franklin, TN. The choice of shifts is Monday thru Friday at 8:30 a.m. to 5:30 p.m. or 1:30 p.m. to 9:30 p.m. and every other Saturday.
General Responsibilities:
- Outreach to members to discuss cost savings opportunities and describe Medco’s unique services and programs
- Outreach to members and their physicians for discussion of any open therapy management issues, including issues related to medication safety, effectiveness and best use of benefits. Issue resolution will address continuity of care, new and refill prescriptions, member requests, physician requests and other issues defined through ongoing practice development.
- Evaluate prescription orders to identify and resolve therapy management issues for members with chronic conditions.
- Provide pharmacy counseling to members on medications, reactions and clarifications.
- Rapidly evolve the specialist pharmacy model to improve performance in health status, clinical outcomes and care objectives of patients through deliberate processes that enable such discovery.
- Provide consultation with and take counseling referrals from internal clinicians and coaches.
- Request consultation with and make referrals to internal clinicians and coaches.
- Partner to pharmacy benefit specialists supporting best use of benefits counseling within each client team.
Required Qualifications:
- Registered Tennessee pharmacist license and license in good standing with Tennessee State Board of Pharmacy.
- Minimum of one year of Pharmacist experience, retail, hospital, or PBM.
Preferred Qualifications:
- PharmD degree or equivalent postgraduate advanced clinical training and practice experience supporting patient counseling, physician relationship management and care coordination with nurses.
- ACCP, ASHP or ASHP certification in practice specialty – diabetes or cardiovascular.
- Continuity of drug therapy management using evidence-based protocols.
- Physician counseling and teaching in a collaborative practice model.
- Expert user with third-party drug information tools and websites for professional and patient communications.
- Clinical residency or fellowship in target specialties with some portion in ambulatory practice.
- Experience in delivering clinical care through a collaborative interdisciplinary team model.
- Minimum of two years clinical practice experience supporting cross-functional and professional role and responsibility communication with nurses, physicians, dieticians and respiratory therapists.
- Formal communication training with mentor experienced in patient counseling calls/contacts - e.g. minimal experience of 500 patients contacts.
- Patient teaching using credentialed training tools and process metrics - e.g. experience of approximately 200 patient teaching sessions.

The Immunology/HIV Specialist Pharmacist Knowledge Manager is responsible for providing assessment, development, documentation, and training services to specialist pharmacists relating to Medco’s Therapeutic Resource Center service, and particularly the Immunology disease state. Candidates for this position should possess therapeutic expertise and an advanced ability to assess needs, design, create, deliver, coach to and evaluate all internal training products. Daily tasks and responsibilities:
- Design, develop and deliver live and web-based therapeutic education to specialist pharmacists linked to specific disease states.
- Conduct needs assessments as required to determine the training needs of pharmacists and maintain ongoing skills development.
- Conduct research projects that lead to presentations at national meetings and publications.
- Interact with subject matter experts to extract required technical information.
- Prepare courseware and documentation using proven instructional design techniques.

Pharmacist Manager, Clinical Practice & Education
OVERVIEW:
The Pharmacist Manager, Clinical Practice and Education is part of the Professional Practices team within Medco. This rewarding role will be focused on the development and delivery of clinical education programs for Medco pharmacists. This is a high visibility, demanding position which will drive medication safety by reviewing, recommending and revising DUR alerts for Medco book of business. Important initiatives in this role include:
- Review and develop clinical content for DUR alerts, dispensing practice guidelines, patient and physician communications, and pharmacist support materials.

Primary Responsibilities:
- Review FDB modules and make recommendations for Optimal Rx programs, including pediatrics high dose, geriatrics high dose, maximum quantity per day, and minimum quantity per day alerts.
- Evaluate literature and provide recommendations for weekly clinical maximum daily dose (MDD) review and existing MDDs. Analyze potential impact and coordinate changes with Professional Practices, Drug Strategy Team, and Integrated Drug File.
- Analyze prescription data to identify opportunities for DUR rules enhancements to improve clinical effectiveness and operational efficiency and write recommendations for modifications to the Clinical Rules Group.
- Support ongoing clinical performance evaluation of pharmacists in the mail service, call center, and specialty pharmacies through retrospective DUR reviews, calibration calls, etc.
- Review and develop clinical content for patient letters, patient newsletters, patient counseling call scripts, physician guidelines, physician letters, and physician faxes for Call Centers, Specialty Pharmacy, Health Management, and DUR Programs.
- Participate in review of medication errors, DUR rules, patient package inserts, auxiliary labels, etc.
- Provide clinical support and expertise for special projects and internal departments including: Legal, Professional Practices, Health Care Operations, Account Management, and Clinical Product Management.
- Develop and provide clinical education programs (didactic lectures and role-plays/case studies) for pharmacists covering related pharmacotherapy and pathophysiology topics as needed.
- Provide support for the Clinical Consultation Service which provides drug information and drug therapy decision-making support to the staff pharmacists, supervisors, and directors of pharmacy practice in the mail service, call center, and specialty pharmacies.
- 10 to 25% travel.

Required Qualifications:
- Registered Pharmacist license and license in good standing with State Board(s) of Pharmacy.
- Candidates licensed in more than one state will be given preference.
Preferred Qualifications:
